What are Home Care Packages (HCPs)?
Home Care Packages (HCPs) program provides government-funded home care across four levels of support to help older people keep their independence and lifestyle, and live in their own home for longer.
There are four levels of HCPs to meet different needs and circumstances.
What services can I receive with my package?
We offer individualised aged care support services that help you address your needs and achieve your goals so that you can live the life you want.
Home and personal supports
Help around the house, including cleaning and gardening.
We can support you in your day-to-day life, including showering, shaving and dressing.
Social and community connections
Stay connected to what’s important to you. Whatever your goals are, we can support you to meet new people, join social groups, visit a local cafe or spend time with friends and family.
Independent living products
We can support you to find assistive products to increase your independence and safety around your home and in your day-to-day activities.
Nursing
We offer nursing services including medical care, health monitoring, and support with medication at home.
Technology support
Technology is important to stay connected with your loved ones.
We can support you to learn how to use a smart device, video conference, how to connect to the internet and participate in virtual activity groups.
Transport services
We can support you to go out and about in the community and get to your appointments.
Home Care Packages - Level 1 to 4, what does it mean?
Because everyone’s needs are different, there are four levels of HCP. Your level will depend on what your assessed needs are (the amount and frequency of support you require for example).
Level 1 - Basic support needs
With HCP level 1, you canget a little extra support to help you stay independent in your own home.
This might include one or two visits a week from a support worker to assist
you with cleaning and tidying around the house, supporting you with your personal health, such as grooming and dressing, or getting some help with meals, shopping and transport.
Level 2 - Low support needs
Similar to level 1, a low level of support to assist you with day-to-day tasks multiple times a week, in your home or in the
community.
The supports and services you can receive are different for each individual: they will depend on you, what you want and what your goals are.
Level 3 - Intermediate support needs
We can support you to find assistive products to increase your independence and safety around your home and in your day-to-day activities.
Level 4 - High support needs
We offer nursing services including medical care, health monitoring, and support with medication at home.
